loader: gh1014- Fix physical device dispatch table
Instead of initializing dispatch tabled for up to *pPhysicalDeviceCount devices, initialize
always all dispatch tables.
This fixes corruption _after_ VulkanCTS dEQP-VK.api.info.instance.physical_devices test.
