layers: Plumb function name/idx through swapchain
PreCallValidateCreateSwapchainKHR is now used to validate
CreateSwapchain as well as CreateSharedSwapchains. Plumbed the
functionName down through the validation code so users can see the
faulting API and the bad swapchain index.
